Methandienone is an orally active derivative of testosterone, 17 alpha alkylated so that it survives the liver, and it aromatises readily into oestrogen. Those two properties define the entire experience of it. The alkylation is what makes it hepatotoxic. The aromatisation is what produces the rapid weight gain, the puffy appearance, the blood pressure rise and the gynaecomastia risk, because a substantial share of what appears on the scale in the first fortnight is water held by rising oestrogen rather than muscle laid down by rising androgen.

Three harms dominate. What methandienone does reliably is increase body weight and strength quickly. What it does not do is make most of that weight muscle.
